For hospitals

Stay connected with patients after they go home.

PonaPath gives your ward teams a simple way to follow up discharged patients by SMS and to act on the replies that need attention, under your hospital's own clinical protocols.

Roles

Who does what

PonaPath is a healthcare technology provider. Clinical decisions stay with the hospital.

The hospital

  • Decides which patients and wards take part, and obtains each patient's consent.
  • Approves the clinical protocols: which replies need attention, response times and escalation.
  • Names the nurses and contacts responsible for reviewing and escalating cases.
  • Makes every clinical decision and records the outcome.

PonaPath Solutions

  • Sends the agreed SMS check-ins and receives patients' replies.
  • Puts replies in one worklist, prioritized and assigned as the hospital's protocols set.
  • Sends the escalation alerts the hospital configures.
  • Sets up the system, trains staff and provides technical support.
For your team

What a ward team works with

1

Patient enrolment at discharge

The ward nurse records the patient's SMS consent and contact details at discharge. The patient receives a discharge message with the hospital's phone number.

2

Scheduled SMS follow-ups

Check-ins go out on the days the hospital agrees. Patients reply with one number from any phone; no app or internet is needed.

3

Nurse review of patient responses

Replies arrive in one worklist for the hospital's nurses, prioritized by the hospital's clinical protocols and assigned to a responsible nurse.

4

Coordinated escalation and documentation

Unanswered cases are escalated to the contacts the hospital names, and every case is closed with a recorded action and outcome.

Getting started

What a hospital needs to take part

  • A clinical lead who reviews and approves the follow-up workflow.
  • One or two wards willing to enrol patients at discharge.
  • Named nurses to review replies, and on-call contacts for escalation.
  • Agreement on patient consent, data protection and how the pilot will be evaluated.

Proposed, not yet agreed. PonaPath is preparing its first hospital pilot. Its scope, wards and duration are agreed with the participating hospital before anything starts.
