How it works

Simple for patients. Structured for the hospital team.

PonaPath links a patient at home to the hospital team that discharged them. The patient uses ordinary SMS; the hospital team works from one worklist, under its own clinical protocols.

The four functions

From discharge to a recorded outcome

1

Patient enrolment at discharge

The ward nurse records the patient's SMS consent and contact details at discharge. The patient receives a discharge message with the hospital's phone number.

2

Scheduled SMS follow-ups

Check-ins go out on the days the hospital agrees. Patients reply with one number from any phone; no app or internet is needed.

3

Nurse review of patient responses

Replies arrive in one worklist for the hospital's nurses, prioritized by the hospital's clinical protocols and assigned to a responsible nurse.

4

Coordinated escalation and documentation

Unanswered cases are escalated to the contacts the hospital names, and every case is closed with a recorded action and outcome.

  1. The patient receives a short SMS check-in on an agreed day.
  2. They reply with one number: 1 = doing well, 2 = I have a question, 3 = please call me. Written messages are accepted too.
  3. They get an acknowledgement. If they report a warning sign or ask to be called, they are told at once to go to the nearest hospital or call 999 / 112 if they feel very unwell.
  4. They can reply STOP at any time to stop messages.

  2. The case is assigned to the responsible nurse or team.
  3. The nurse reviews the case and takes an action, such as a phone call or advice to attend clinic.
